I certify that all of the above is true & understand that ANY false information may result in revoking the adoption. I understand and agree as the adopter that this cat is never to be abandoned, sold, or disposed of in any way or be surrendered to an animal shelter. In addition, Operation Topcat retains the right to perform a home check to verify a proper environment for the adopted cat. I also understand and agree to keep the cat indoors at all times and provide vet care as needed. In the event that I can no longer keep the cat, I agree to notify Operation Topcat allowing sufficient time for Operation Topcat to HELP find a new home. I understand I may receive a phone call or email from a member of Operation Topcat as a follow up to the adoption.
